United states Navy Lieutenant Commander John S. McCain, III, an A4E Skyhawk pilot aboard the USS Forrestal, “wet-starts” his aircraft, in an effort to startle the pilot, of an F4 Phantom directly behind him.  The flame launches an M34 Zuni rocket that releases a half-ton bomb that starts a fire that kills not only the F4 pilot but also 167 Sailors.

       Postscript: As a result of his horseplay, McCain (the son and grandson of admirals) was quietly transferred to the USS Oriskany.

       NOTE: Before arriving aboard the Forrestal, McCain lost three high performance aircraft on routine training flights, but still managed to keep his wings.
